If you want to get registered please read the additional information below.

For the first visit, we ask you to do, and bring with you the following:

• 10 Minutes before the actual appointment, sign in at the counter in our practice because we need you to fill out some forms, such as a health questionnaire.
• Your dental history. This you should ask your previous dentist yourself and bring them to the first visit or send them directly to us. By Digital photos please send us an email.
• A valid ID or a passport.
• A health insurance card.
• If you are not able to make it to the appointment, please inform us at least 24 hours in advance.

If you are living in the Netherlands for just a short period of time or/and if you do not have a regular Dutch health insurance, it is possible we ask you to pay the treatments right away.

To make an appointment there are two options:
